The authors present a case of a young man with extragonadal germinative tumor presented with hemoptysis and bronchopneumonia as the first symptom. Clinically asymptomatic complication of the disease was a large inferior caval vein thrombosis. The presented case demonstrates the need of an interdisciplinary cooperation in both the diagnostic process and the following treatment of diseases less common in a
pneumology.

We present a case-report of a 20-year-old female with 8-years history of diabetes mellitus type 1 (DM 1) managed with an insulin pump. She was admitted to an intensive care unit with hyperglycemic ketoacidotic precoma resulting from the insulin pump failure caused by the canulla obstruction due to the skin abscess. At the admission the chest X-ray revealed an infiltration in the apical segment of the right lower lobe. The infiltration matured into the abscess on the next day and did not resolve after oxacilin administration. The only positive test of sputum examination was PCR evidence of Mycobacterium tuberculosis. Culture positivity was proved later on. PPD test was 12 mm. Standard antituberculotic treatment was successfully completed after 6 months.
By presenting the case we want to point to:
1. Seriously decompensated DM can result in rapid development of cavitatory TBC with an unusual occurrence in lower lobe.
2. TBC in seriously decompensated DM may have sings of either a rapid progressive primary or an early postprimary infection even in a vaccinated
person.

Bronchial asthma is one of the most common chronic diseases worldwide. Approximately 300 millions of people suffer from it. Asthma does not avoid sportsmen; top level sportsmen suffer from asthma five times more frequently than other population. Intense physical stress very often causes asthmatic problems and sometimes it can be the only inducing factor of asthma worsening.
Asthma pharmacotherapy in top level sportsmen is based on the same general principles; however specific doping issues are
involved.

The author presents a case history of a less frequent cause of breathlessness, that resulted in a severe respiratory insufficiency with extreme secondary polyglobulia and pulmonary arterial hypertension. It was caused by a congenital heart defect with foramen ovale apertum, that due to a traumatic rupture of a trabecular ligament of a tricuspid valve changed into a right-left short circuit defect with all hemodynamic
consequences.

Mucoceles of paranasal sinuses are not rare. Typically, they are growing slowly. They occur most frequently in a connection with either chronic rhinosinusitis or previous intervention in the paranasal sinus. We present two cases of a mucocele with rapid development of clinical signs. We describe standard diagnostic methods and different surgical approaches depending on the size of the mucocele and the presence of
complications.

The authors present a rare case of the first symptom of the hepatocellular carcinoma presented as sternal metastasis in a patient with diagnosed hepatitis C who has not been adequately supervised.Inadequate assessment of the first symptoms of the disease resulted in a fatal
outcome.
